Here’s how it works...

Step 1: Create your form on Google Forms, SurveyMonkey, TypeForm, or Wufoo

Step 2: Edit the Automated Workflow for the Group you want to receive this form.

Step 3: Paste the link to the form in your Automated Workflow step and click Save Changes.
